Sex Therapy vs Couples Therapy: Which One Do You Need?
Most couples in Miami try general counselling first. Here is how to tell when the sexual part of the problem needs its own specialist.

The short answer
Couples therapy treats the relationship — communication, conflict, resentment, repair. Sex therapy treats the sexual problem directly, with a structured assessment of what happens in the body and a set of graded exercises done at home. If sex is one symptom of a struggling relationship, start with couples work. If the relationship is broadly fine and the sex is the problem, general counselling will usually circle it politely for months.
The difference is training, not care
This is not a claim that couples therapists are worse. It is that the two disciplines were built for different jobs. A couples therapist is trained in the dynamics between two people — how conflict escalates, how attachment injuries form, how repair happens. A sex therapist is trained in that plus sexual function: arousal, desire, pain, orgasm, the mechanics of what actually occurs and stops occurring.
When a couple with a specific sexual difficulty is seen by a clinician trained only in the first, the sessions tend to drift toward communication about sex rather than the sex itself. Couples describe leaving with a better vocabulary for the problem and no change in the problem.

Which to start with
- Choose couples therapy first if there is active contempt, an ongoing affair, or you cannot have a twenty-minute conversation without it turning. Sexual homework does not survive that environment.
- Choose sex therapy first if you generally like each other, the friendship is intact, and the sexual difficulty is the thing eroding it.
- Choose sex therapy if a medical workup came back normal and nobody gave you a next step.
- Choose either, and expect the other, if you honestly cannot tell. A first-session assessment is meant to sort exactly this, and a certified sex therapist can do both jobs.
If you already did a year of the wrong one
This is extremely common and it is not wasted time. Couples who have done general work usually arrive with skills that make sex therapy faster: they can hear feedback, they can stay in a difficult conversation, they have stopped scoring points. What they were missing was the specific part, and that part is teachable in a fraction of the time when the relational groundwork is already there.
Frequently asked questions
Can one therapist do both?
A certified sex therapist is a licensed therapist first, so yes — the sexual work sits on top of standard relational training. The reverse is not true: a couples therapist without sex therapy training has not covered sexual function.
Do both partners have to attend?
Not always. Plenty of sexual difficulties are treated with one partner in the room, particularly where the maintaining factor is anxiety or a personal history. Joint sessions get added when they help.
How long does sex therapy take?
It depends entirely on what is maintaining the problem, and any specific number quoted before an assessment is a sales figure rather than a clinical one. What is fair to say is that a focused sexual difficulty is often shorter work than open-ended couples counselling, because the target is narrower.
